Thank you for agreeing to participate in the Massachusetts Gambling Impact Cohort (MAGIC) study! You are one of 3,100 participants in this study. The present survey is very similar to the one you filled out a few years ago, but with some additional questions. Each year around this time we will contact you again and ask you to participate. Most people will finish the survey in 20 - 40 minutes. As compensation for your time we will be paying you $50.
